Enumeration forms used Two long form questionnaires: F1 for households and residences, and F2 for individuals. One questionnaire with dwelling information, social and demographic characteristics and fertility and contraceptive practice. Long form questionnaire SP80-S containing housing and individual questions. One questionnaire with dwelling information and social and demographic characteristics of individuals. Long form questionnaire SP90-S containing houseing and individual questions distributed to 5% of households. Sample design Data are derived from the .038 sample of enumeration blocks that received the long form questionnaire. These clustered data were stratified by type and population size of regency to create the microdata sample. Sample of households in 770 clusters throughout country. Three provinces were not fully covered: East Nusa Tenggara, Maluku and Irian Jaya. Data are derived from the sample of census blocks that received the long form questionnaire. The sample of census blocks that received the long form was stratified by urban-rural status, with special census blocks containing large numbers of persons in institutions comprising a separate stratum.
