6.13. Question 12: Place of birth
6.13.1. Concept and definition
Place of birth is defined as the province where the mother lives when a person is born. If a child is born in a different province from his/her mother, for instance, if the child is born in a hospital that is located in a different province from his/her mother's province, the child's place of birth still follows his/her mother's province.

For example, a mother who lived in Bekasi gave birth to a child in a hospital that is located in DKI Jakarta; therefore the child's place of birth is Bekasi. If a person is born in a province that is now divided into two provinces, then the place of birth should be recorded as the new province. For instance, a person is born in Bengkulu, which was still a part of South Sumatra province at that time, but his/her place of birth would still be recorded in Bengkulu not in South Sumatra province, even though Bengkulu did not exist yet.

6.13.2. Procedure
Ask this question:
"Where were you born?"

If the province of the place of birth is known, tick the appropriate province code.

Example:
Amat was born in Semarang. Now, Semarang is located in the Central Java province. The code for Central Java province is 11, therefore put a dash (-) in the number 1 for the tens box and the number 1 for the ones box.

Column (2) Place of birth
Place of birth place is the place where someone is born. Usually the place of birth of a person is the place where the person's mother resided when the person was born.
Ask the question as follows:
"Where was (mention name of household member) born?"


Then ask and write down the name of the regency/municipality where the person in question was born. If born in a regency, strike out "Kod" in column (3). Conversely, if born in a municipality, strike out "Kab".

Column (4) doesn't need to be filled in:

- If someone is born abroad, then write the name of the country where he/she was born.
- If one is born while on a trip aboard a ship from another country, then the place of birth will be the country whose flag the ship flies.

A. Block VII: General information on household members
1. The objective of this block is to record information on items such as sex, age, schooling status, and activities conducted during the past week for every household member, starting with the member with serial number "01" and continuing to the last household member.

2. Method of filling out the form
Circle one of the codes corresponding to the respondent's answer, and then write the code in the box on the right.


B. Block VII. A: Persons of all ages


Question 7: Place of birth
Fill in the name of the province and regency/municipality of the respondent's place of birth. Province/regency/municipality of the place of birth is the province/regency/municipality of the mother's place of residence at the time the respondent was born. For example, a mother resides in Bandung (West Java); she gives birth to a child in Solo. If the mother and child return to Bandung (West Java) in fewer than 6 months, then the child's place of birth should be recorded as Bandung (West Java). However, if the child remains in Solo for 6 months or more, then the child's place of birth should be recorded as Solo (Central Java).

If there has been a change in the name of the province/regency/municipality, then the recorded place of birth should be the name of the area at the time of the enumeration. For example, A was born in Cakung sub-district, Bekasi regency, province of West Java. Because of the expansion of DKI Jakarta [DKI Jakarta is the special capital region Jakarta and is a city/province], at the time of the enumeration, Cakung sub-district is included as part of DKI Jakarta. Thus, the place of birth of A is the province DKI Jakarta, not West Java.

Question 8: Place of birth
Fill in the province where the respondent was born. The birthplace is the province where the respondent's mother gave birth to the respondent. Example: if a mother lives in Bandung (West Java) and gives birth to a child in Solo (Central Java) and if the mother and child return to Bandung after fewer than 6 months, the child will be registered as born in the Province of West Java. If the child stays in Solo for more than 6 months, the child will be registered in Province of Central Java.

6.5 Block V. General characteristics of household members
This block is used to record personal characteristics such as sex, age, mobility of living, attained education, school participation and the daily language used by each household members, starting from the first serial number of the household member until the last one.


Block V is asked to all household members
In order to fill in this block, the enumerator should try to directly interview the respondent.

Fill in the province and municipality/regency where the respondent was born. The birth place is the province and municipality/regency where the respondent's mother gave birth to the respondent. Example: if a mother lives in Central Jakarta and gives birth to a child in the Regency of Malang (East Java), and if the mother and child return to Jakarta during the following 6 months, the child will be registered as born in Jakarta. If the child stays in Malang for more than 6 months the child will be registered in the Regency of Malang (East Java).

Question 04: Regency/municipality and province of birth
Regency/municipality and province of birth pertains to the residential address of the respondent's mother when giving birth, under the condition that the address is the permanent de jure population address and not just the address at the time of birth.

Please write down the regency/municipality and province of birth into the correct boxes provided. The area codes of the area names will be filled in by the supervisor of the enumerator.

Note: When it is found that the respondent was born abroad, please write down the country name where the respondent was born and put the code "96" into the boxes of regency/municipality and province.

Question 607: Place of birth
Fill in the province and regency/municipality of the respondent's place of birth. Place of birth is the province and regency/municipality of the respondent's mother when she gave birth to the respondent. For example, a mother resides in Central Jakarta (DKI Jakarta); she gives birth to a child in the Regency of Malang (East Java). If the mother and child return to Central Jakarta (DKI Jakarta) in fewer than 6 months, then the child's place of birth should be recorded as Central Jakarta (DKI Jakarta). However, if the child remains in the Regency of Malang for 6 months or more, then the child's place of birth should be recorded as Regency of Malang (East Java).

The administrative boundaries used in this survey are the latest administrative boundaries. Write the name of the province and the name of the regency/municipality of the respondent's place of birth according to the boundaries used in this survey.

Example: Andika was born in 1990 in the Tangerang Regency, West Java Province. If Andika is selected in the sample, then to fill out Q 607, the province is Banten and the regency is Tangerang. This is due to the changes in the administrative boundaries where the Tangerang Regency became part of Banten Province.

Note: If the respondent's place of birth was abroad, write the name of the respondent's place of birth.

Question 205: Place of Birth
Ask and write the name of the province and regency/city where the household member was born. The enumerator is only responsible for clearly filling in the name of the province and regency/city. The codes for the province and regency/city will be entered by the Team Coordinator.
Place of birth is the province and regency/city where the mother lived at the time of the household member's birth. The administrative boundaries of the regions where the respondent was born should be the latest ones which are used at the time of the enumeration.

Example:

1. A mother resides in East Aceh (Province NAD); her child is born in the city of Medan (Province of North Sumatra). Then the child's place of birth is considered East Aceh (Province NAD).
2. Andika was born in 1990 in Manokwari. In 1990 Manokwari was in the Province of Irian Jaya. For Q205 enter the Province for Andika as West Papua and the Regency as Manokwari, because since 1999 the Regency of Manokwari became part of the Province of West Papua.

3. Tuti was born in 1985 di Cimanggis, Regency of Bogor, West Jawa. In 1985 Cimanggis became part of the Regency of Bogor. For Q205 fill in the place of Tuti's birth as Province West Jawa and City Depok, because since 2004 Cimanggis changed administrative districts and became part of the City of Depok.

If a household member was born abroad, write the name of the country where the household member was born in the space for province/state and strike out the word "province"; enter a dash "in the space for the regency/city. A code for the country will be entered by the Team Coordinator (See Book 7: Codes for Ethnicity, Language, and Administrative Area).

The enumerator must take care in writing the name of the province/country or the regency/city so that he/she does not write in the boxes provided for the codes. If the name of the place of birth is long, use a common abbreviation or just write as much of the name as possible so that the writing does not extend into the boxes.
For Example: BARITO SEL (Barito Selatan), KALTIM (Kalimantan Timur), SULBAR (Sulawesi Barat), MALUT (Maluku Utara), TAPUT (Tapanuli Utara), TOBASA (Toba Samosir), OKU (Ogan Komering Ulu). If there is an acronym or an abbreviation that is standard or standard in the region, then use that.

If the enumerator knows that the name of the regency/city given by the respondent is not the name of a regency/city, then he/she should ask whether (mention correct name of regency/city) is the one intended by the respondent.
