If there are more than 10 members in the household, use additional questionnaire(s). When this is necessary, tick the box in the bottom right corner and change the person number (1, 2, 3 ... in column a in the questionnaire) to 11, 12, 13 ... in the second questionnaire, 21, 22, 23 ... in the third questionnaire, etc.

Question 5: Where was [the respondent] born?
The purpose of this question is to get a measure of "life-time" migration.
Birth place refers to the place where the birth actually occurred. For those born in Lao P.D.R, enter the code referring to the relevant province/district and for those born outside Lao P.D.R, the code of country of birth.
A list of the districts and countries is shown in Appendix 2 and at the back of each questionnaire. For not known use code "9999".

This question is aimed at collecting information about the migration of citizens from birth to the present.
Place of birth means the place of birth of the head of the household and all household members, which is defined as the district, province, and country where the person was born.
